30-DAY REFUND/EXCHANGE CLAIM FORM

Terms: This form MUST NOT be modified in any way. If you modify this form, we will not be able to process your refund/exchange request. One (1) refund claim form must be returned for each product that you are requesting a claim for. Downloadable products are NOT refundable, they may be returned for EXCHANGE ONLY within 30-days of your original order date. The returned product may be exchanged for another downloadable software product sold by Information Net Source Corp. Please see lists below for other products. Please check the product you would like to exchange in the Yellow Box, and also select the product that you would like to exchange it for in the Blue Box. If you are returning a CD or a Hard Copy (paper) version of a product, please skip this section and proceed to the section labeled "Personal Information".

In order to obtain a refund, you, the customer, must agree to the following terms.

1 - You HAVE deleted ALL instances of the software from any media that it was stored in/on such as, but not limited to a web server, computer hard disk, floppy disk, and email account.

2. - If you ordered a CD or a paper version of our product, you must return, to us, said product along with your refund claim form.

2 - You must immediately stop using the software for any purpose whatsoever.

3 - If applicable, you must keep all programming logic, code structure and source code confidential. You may not reveal this to anyone.

In order to receive a refund or exchange for the software you originally purchased, you must print this form, fully complete the information below, read our terms and conditions, and sign our binding agreement on the last page of this document. When completed, mail this claim form to:

ANY VIOLATIONS OF THIS AGREEMENT MAY LEAD TO CIVIL AND CRIMINAL CHARGES TO THE FULLEST EXTENT ALLOWABLE BY LAW, INCLUDING BUT NOT LIMITED TO COPYRIGHT INFRINGEMENT AND OTHER APPLICABLE LAWS OR TREATIES, AS WELL AS ANY EQUITABLE RELIEF THAT MAY BE NECESSARY IN ORDER TO ENFORCE THIS REFUND AGREEMENT.
